Enrolment at Fairton School

The forms below will need to be filled in upon enrolment.  You may either collect these from school or donwload and print the attached PDFs.

When presenting these to school you will need to bring with you:

* Your child's birth certificate

* Immunisation records 

Please make sure that you have included the emergency contact details and your doctor's details on the enrolment form.
